Supervisor: Community Outreach Mental Health Promotion and Support (COMHPS) Team Leader
Position Summary:
Under the supervision of their Team Leader, the Community Wellness Specialist will provide community engagement, mental health wellness promotion, individual screenings for mental health conditions, brief counseling, and referral, as indicated to licensed mental health professionals. They will work in support of the program’s goals and provide; Outreach Activities, Wellness Activities, Assessment, referral, and linkage activities, and Treatment-adjacent service offerings. The COMHPS Program aims to support New Yorkers with subclinical mental health needs and establish, maintain, and improve individual and community mental health and wellness.
Duties And Responsibilities
Outreach activities toward community members, focusing on targeting youth, young adults, family members, and key support systems. Required outreach activities include:
In-person (e.g., tabling at local events) and digital (i.e., virtual, phone, and text) strategies for reaching and engaging target groups.
Establishing partnerships with local community-based organizations to provide public education about available services.
Wrap-around support (e.g., emergency assistance for youth, young adults, and families who are ineligible for public benefits).
Wellness activities leveraging trained paraprofessionals working continuously in their communities to support mental wellness. Suggested wellness activities include:
Group facilitation, either with or without psychoeducation.
Faith and spirituality activities.
Public education (community level and individually focused).
Organized recreational activities that promote community wellness (e.g., fun runs/walks, creative activities such as arts/crafts and music, or other organized activities).
Assessment, referral, and linkage activities. Required assessment and referral activities include:
Mental Health Outreach Workers will screen individuals for common mental illness symptoms using OMH approved instrument(s) and refer them to known public mental health clinical services as appropriate.
Referrals and linkage will be made by warm call transfer, personal accompaniment or in-person visits, or virtual introductions of those with expressed or assessed need.
Treatment-adjacent service offerings to include:
Brief individual and group counseling to address intense stress, trauma, and/or anxiety.
Family education and counseling.
Academic and vocational support.
Work with individuals, families, and groups by providing counseling services either virtual or in-person that include outreach, emotional support, individual and group counseling, public education, and referrals when needed
Represent program in the community and network with other agencies and partners to ensure needs are met
Present to community groups on various psychoeducational topics
Complete daily log documenting detailed staff-specific tally sheets capturing all outreach, engagement, and counseling activities.
Conduct workshops, outreach, counseling, and education in communities on various psychoeducational topics
Provide treatment adjacent services to community and BPC connected persons
Other related duties as assigned
Requirements:
Ability to communicate with individuals in a professional and empathic manner

Ability to display a professional and positive attitude
Completion of ongoing COMHPS and BPC training requirements
Proficient in Microsoft Office
Qualifications
High School Diploma or GED required
Working knowledge of Trauma-Informed Care
Cultural Competency required
Trained paraprofessional or professional
Must be reliable and able to work independently and understand the importance of maintaining confidentiality
Must be cleared NYS Justice Center through fingerprinting for a Criminal History Records search
Two years of coursework or equivalent experience in social or human services or related fields (social work, human services, community/public health, sociology, psychology, mental health counseling, or related field)
Bi-lingual a plus

Bachelor’s Degree
Candidates of highly impacted communities are strongly encouraged to apply
Relevant years of experience and gained competencies will be considered as strongly as education during the interview process
New York City resident
Familiarity with New York City neighborhoods, communities, and boroughs
Salary: $26.00 to $28.00 / Hourly
