Care Management Manager

The Care Management Manager at San Luis Valley Health will be responsible for ensuring that teams requiring clinical oversight and supervision perform to the highest quality and compliance standards, supporting patients in achieving health outcomes, providing clinically-appropriate care planning, and maximizing top of scope work, while meeting the contract requirements of various programs, through the oversight, daily supervision, and coordination of care management activities within clinical settings. This position manages Behavioral Health (including providers, consultants, and interns), Discharge Planners, Care Managers, and Outreach Care Coordinators.

What You'll Need

  • Licensed Clinical Social Worker, required
  • 3 years of case management, discharge planning, or social work experience, or equivalent required.
  • Minimum 2 years of direct patient care providing therapeutic interventions, required.
  • Experience with clinic or hospital management preferred.
  • Knowledge of ambulatory care, clinic workflow, and team-based care preferred.
  • Experience working in various software platforms, required (Microsoft, Electronic Health Records, databases, etc)
  • Experience with staff supervision, clinical or administrative, required.
  • Solid working knowledge of evidence-based transitions of care services, required.

What You'll Do

  • Responsible for hiring, training, and onboarding; as well as coaching and discipline of team members.
  • Responsible for the clinical supervision and oversight of individual and team to meet licensure, compliance, and quality standards.
  • Responsible for the implementation and compliance of requirements related to grants and contracts, within clinical settings.
  • Responsible for supporting team members on integrating within the multidisciplinary team setting, to work successfully with providers, nursing staff, admissions, etc.
  • Responsible for meeting contract requirements regarding monthly reports, data analysis, and other program documentation expectations.
  • Responsible for attending meetings related to department(s) activities, both internally and externally from the organization.
  • Responsible for facilitating staff meetings, group supervision, and individual supervision. This includes facilitating or coordinating training needs of teams.
  • Ability to work well with partner agencies at meeting community and patient needs.
  • Fosters and promotes positive interactive behavior and communication with all members of the health care team to support a customer service culture.
  • Resolve staffing coverage issues.
  • Travel throughout the valley and outside of the valley for meetings and staff supervision.
  • Meets budgetary requirements for departments.
  • Ensures departments meet strategic expectations regarding access, quality, and patient experience, through the support of daily operations.
  • Supports the development and implementation of additional population health related services, as needed. This may include future grants, peer support services, care management programs, or enhanced care delivery opportunities.
  • Provide clinical oversight to Behavioral Health department, including quality, training, policy development, and outcomes.
  • Provide input on clinical factors for staff and intern performance evaluations.
  • Maintains a current level of clinical practice by selecting appropriate avenues for professional growth and routinely seeks continuing education according to best practice.
  • Evaluates goal attainment and revises goals on an on-going basis.
  • Provide direct clinical supervision to all clinical staff and interns.
  • Provides direct patient care, as available (includes Behavioral Health Therapy, care management, discharge planning, etc.)
