If you’re headed to the emergency room, it can often be during a stressful situation. So, it can be helpful to understand who and what you might see during your visit to take some of the guess work out of the situation. Garrett Oschner, emergency department tech at Northern Nevada Sierra Medical Center, is sharing some of his tips for navigating an ER visit. “Although some conditions may take precedence due to severity, it’s important to know that every person who walks into the ER will be cared for as quickly as possible,” he said. Once you’re taken back, a team of professionals will be involved in your care. During your visit, you may see: • Triage nurse: Evaluates the severity level of your condition. • Doctor, PA or APRN: Performs an initial screening exam. • Emergency technician: May perform an EKG, wound care, splinting and other duties. • Radiology technician: Performs needed imaging. • Phlebotomist: Will draw your blood for any needed testing. • Doctor: Responsible for determining your care plan. • Nurse: Responsible for providing your care.

DOES YOUR CARDIAC ARREST ROOM EVER LOOK LIKE THIS? 🚑 When EMS notifies us of an incoming full arrest, it triggers a well-choreographed response. 👨⚕️ The team springs into action, setting up the room with essential equipment like the crash cart, glidyscope, IV start kit with blood tubes, CPR stool, and preparing the necessary documentation on code sheets. This preparation is crucial because when the patient arrives, the room is often bustling with activity. ➡ Multiple paramedics rush in with the patient, accompanied by a team of nurses, doctors, ER techs, and respiratory specialists managing the ventilator. ➡ In such a crowded and high-stakes environment, having clear roles assigned to each team member is very important.. Without defined responsibilities, the code can quickly descend into chaos, leading to potential errors or missed tasks. ➡ That's why as a charge nurse, or when the code is in my room, I emphasize the importance of everyone having a specific role. I often remind the team that if they don't have a designated task, it's better to step back and allow those with defined roles to focus on their responsibilities. ✔ This approach helps maintain order and ensures that every aspect of the code is addressed efficiently. ✔ Speaking from personal experience, I find myself most engaged and effective when I'm directly involved in patient care during a code. Tasks like starting IVs, administering medications, and coordinating with the team to optimize patient outcomes are where I feel most in my element. ❗ On the other hand, while I understand the critical importance of documentation during a code, it's not my preferred role. Balancing the urgency of the situation with meticulous recording can be challenging, requiring a focused mindset despite the surrounding chaos. CPR (Cardiopulmonary Resuscitation) CPR is a life-saving emergency medical procedure performed on someone who is experiencing cardiac arrest or has stopped breathing. CPR is designed to keep the blood and oxygen flowing to vital organs until professional medical help can arrive. It involves a combination of chest compressions and rescue breaths to maintain circulation and oxygenation in the body. Here's a basic guide on how to perform CPR on an adult: Check the scene: Ensure that the area is safe for both you and the victim. Look for any potential dangers or hazards before approaching the person. Check for responsiveness: Tap the person and shout loudly, "Are you okay?" If the person does not respond and is not breathing or not breathing normally, immediately call for emergency medical help (call your local emergency number). Open the airway: Gently tilt the person's head back and lift the chin up to open the airway. Check for breathing: Look, listen, and feel for breathing. Watch for chest movement, listen for breath sounds, and feel for exhaled air on your cheek. Begin chest compressions: If the person is not breathing or only gasping, start chest compressions. Place the heel of one hand on the center of the person's chest (between the nipples), and place the other hand on top of the first hand. Lock your elbows, position your shoulders directly above your hands, and use your body weight to compress the chest downward at least 2 inches (5 centimeters) deep and at a rate of about 100-120 compressions per minute. Provide rescue breaths: After 30 chest compressions, give two rescue breaths. Pinch the person's nose shut, cover their mouth with yours, and give a breath that makes their chest rise visibly. Each rescue breath should be about 1 second long. Continue CPR: Repeat cycles of 30 chest compressions followed by 2 rescue breaths until emergency medical help arrives, the person starts breathing on their own, or you are too exhausted to continue. It's essential to remember that CPR is a crucial skill, and proper training is recommended for everyone. If possible, take a certified CPR course to learn the correct techniques and get hands-on practice.
