Chiropractors often see patients with persistent symptoms who have undergone extensive medical assessment and care. Following is a case posted online by an attending doctor. A 13-year-old boy fell and injured his knee. He saw pediatric orthopedists and a rheumatologist. Fracture was not identified. Extensive care was rendered without relief. On viewing the MRI, I noticed linear increased T2 and decreased T1 signals through the proximal tibial epiphysis and metaphysis. A neuroradiology colleague confirmed that this is consistent with a Salter-Harris Type IV fracture. It is likely the patient’s recalcitrant pain was due to the unidentified fracture and inappropriate care. I would recommend re-evaluation by a qualified medical pediatric orthopedist. Some times simple therapies also work wonders This 70 yr old patient who was a follow up case of mesentric ischemia post SMA stenting being treated for loss of appetite with abdominal pain and had almost completely stopped eating anything at home became unconscious and was admitted to icu totally unresponsive with almost no evidence of brain activity except his pupils reacting to light.Over the next 3 days the entire team including Gastro surgeon,neurologist,gastroenterologist & intensive care team wrecked our brains trying to find a cause for his unconsciousness as all his tests were negative except high blood ammonia.Unlike most patients with high blood ammonia levels his liver functions were normal as were the other common causes. After much literature search,we chanced upon some rare reports of starvation being a cause of high ammonia levels.Over the next few days we reduced his ammonia via regular slow low efficacy dialysis & ensured meticulous correction of his Nuitrition. I am curious if any clinicians have insight into literature/studies exploring inclusive care for patients with neurodiversity/sensory disorders/autism spectrum disorder who are receiving radiation therapy. It is heartbreaking to see patients in distress due to the mask and prolonged time on the table. I know that sometimes sedation is an option, however, in the interest of reducing potential risk to the patient (poor response to anesthesia, etc), I wonder if there are any holistic support measures we can provide. I'm passionate about inclusive care and taking steps to avoid additional stressors. #radiationoncology #autismawareness #neurodiversity #inclusionmatters

Multiple Sclerosis’ Dirty Little Secret 💥 ⚡ Medical gaslighting ⚡ when healthcare professionals downplay, dismiss, or ignore a patient’s symptoms or wrongly blame their concerns on psychological factors. At best, it can cause a person to leave their physician’s office feeling frustrated, confused, or full of self-doubt. At worst, it can cause a delay in a patient receiving the correct diagnosis and treatment, which can lead to irreversible damage. Why MS ❓ MS is a notoriously tricky disease to diagnose and manage. Presently, there is no single biomarker to confirm an individual has MS, so it often takes dedicated medical sleuthing on the part of a neurologist to determine a diagnosis. An MRI scan may show the presence of lesions and the need for spinal fluid testing, whereas a lumbar puncture can point to immune system activity around the brain and spinal cord. But neither of these is definitive. Another hurdle is that MS symptoms vary from person to person; they wax and wane over time, and many are invisible (eg, fatigue, pain, anxiety, cognitive dysfunction), meaning they are difficult to measure and manage. In addition, many other neurological conditions mimic MS, including migraine, cerebral small vessel disease, systemic lupus erythematosus, or autoimmune conditions such as neuromyelitis optica. Ways for patients to avoid it because the consequences can be so severe. Push back 👊 Prepare for medical visits. ✍ Ask to record the consultation ⏺ Bring along a care partner to give you emotional support. 🙎♀️ Focus on the key issues 📍 Pin down the next steps 📌 Our role as HCPs : ✔ Recognize the existence and impact of medical gaslighting in the MS community, acknowledging its potential to hinder accurate diagnosis and timely treatment. ✔ Understand the prevalence of medical gaslighting, especially among women with MS, and empower patients by proactively addressing concerns, helping them be prepared for medical visits, and seeking support to navigate potential dismissals or delays in care. ✔ Advocate for change within healthcare practices by encouraging open communication, recording consultations, seeking second opinions, and fostering a partnership-based approach between patients and healthcare providers.
