80% of first diagnoses of HF are found on emergency admission in hospitals, where 1 in 10 will die and1 in 3 will die in the year after a first HF diagnosis in hospital. Mostly, these are avoidable deaths. Finding symptoms of HF will lead to earlier diagnosis, reduce these acute admissions and ultimately save lives. #IWasFound #FindMe #25in25 #DetectTheUndetected #FreedomFromFailure

This month, Monogram Health is bringing awareness to the importance of heart health. Heart disease has been the leading cause of death in the United States for 75 years. Half of all patients with CKD stage 4 of 5 have cardiovascular disease and it is responsible for nearly half of all the deaths in this population. Integration between specialists is essential for these patients. Far too often, that doesn’t happen in an increasingly complex and disconnected healthcare system. Proud of our doctors and nurse practitioners who are taking the complexity out of care, providing coordinated, comprehensive multispecialty care in the comfort of a patient’s home. #MonogramHealth #AmericanHeartMonth #HeartHealth #HealthyHeart #HeartAwareness #LoveYourHeart #CardiovascularHealth #HeartMonth #HeartDiseaseAwareness #HeartWarrior #valuebasedcare #nationalspecialtyprovider #evidencebasedcare #inhome #polychronic #ckd #eskd #caredelivery #caremanagement #providersofcare #caretransformation #improvedoutcomes #quality #costcontainment #patients #bendingthecostcurve #MLR
