What happens if you don't respect anatomy at the time of total hysterectomy? ❗ Do not underestimate the risk of prolapse. ❗ Respect the anatomy and attempt to restore it after removing the uterus even if there is no obvious prolapse present. ❗ Even in our day there are still people "treating" uterine prolapse with simple hysterectomy. ✳ Remember, the cervix is the anchoring point of the pubocervical fascia, the cardinal ligaments and the uterosacral ligaments as an extention of the rectovaginal fascia. ⚠ Case of the week! Control examination 6 weeks after TVT placement. With routine pelvic floor sonography we are used to check: 1. 𝐓𝐚𝐩𝐞 𝐝𝐢𝐬𝐭𝐚𝐧𝐜𝐞 𝐟𝐫𝐨𝐦 𝐛𝐥𝐚𝐝𝐝𝐞𝐫 𝐧𝐞𝐜𝐤 (aim to be at 50-70% total urethral length), 2. 𝐓𝐚𝐩𝐞 𝐝𝐢𝐬𝐭𝐚𝐧𝐜𝐞 𝐟𝐫𝐨𝐦 𝐥𝐮𝐦𝐞𝐧 (aim to be about 3-5mm but even more may be acceptable in cases of hypermobile urethra) and 3. 𝐓𝐚𝐩𝐞 𝐟𝐨𝐫𝐦 (straight in relaxation and C during Valsalva). All of the above are typically done in B-mode. What about 𝐭𝐚𝐩𝐞 𝐢𝐧 3-𝐝𝐢𝐦𝐞𝐧𝐬𝐢𝐨𝐧𝐚𝐥 𝐬𝐩𝐚𝐜𝐞? Well, do have a look!
