Position Overview:
We are seeking a dynamic and experienced Lead Case Manager for our Mental Health team. The Lead Case Manager will play a pivotal role in group, individual and family counseling at our center, ensuring the highest standards of care are maintained and innovative approaches are integrated into our programs. This position oversees Case Manager and directly manages a client caseload.

Essential Duties and Responsibilities:
Uphold and Enforce Standards, Policies, and Ethics
Ensure compliance with The Forge Recovery Center’s standards, policies, and code of ethics
Uphold client rights and professional conduct
Intervene in crisis situations as necessary
Clinical Competence
Demonstrate proficiency in TAP21 Counselor Competencies
Utilize agency assessment tools in KIPU for comprehensive client evaluations
Identify and prioritize patient needs and issues
Person-Centered Care Planning
Develop individualized care plans focusing on client-reported goals
Conduct therapy sessions using evidence-based practices
Document client progress in SOAP notes
Prepare effective discharge plans and transition notes
Collaboration and Communication
Support and train Case Managers to help them improve their skills and knowledge
Coordinate with health insurance for utilization reviews
Participate in clinical treatment team meetings
Provide accurate assessment and referral services
Communicate effectively with probation officers, lawyers, and family members
Professionalism and Compliance
Adhere to ethical and legal standards
Ensure accurate and timely clinical documentation
Maintain awareness of cultural competency and client satisfaction
Prioritize safety of staff and clients
Education, Experience, & Skills:
Case Manager Credential. RADT minimum, CADC I, SUDCC II, CATC II or above preferred
3-5+ years of Case Manager Experience
Passion for Client focused work and continuous improvement
AA or BA degree in Human Services or Behavioral Health, or equivalent experience
High computer literacy skills
Additional Certificates & Licenses:
CPR/First AID
Valid Driver’s License
Other Requirements:
Negative TB test results within past year
Medical clearance to work within past year
Able to work evenings when needed
Job Status: Full-Time
Location: On-site in Santa Ana
