Role Description
This is a full-time on-site role for a HIV Nurse Case Manager at Holyoke Health Center, Inc. located in Holyoke, MA. The HIV Nurse Case Manager will be responsible for utilization management, nursing, medicine, and case management.
Qualifications
Experience in medical case management
Strong utilization management skills
Excellent nursing and medical knowledge
Ability to work in a fast-paced environment and manage multiple tasks
Strong communication and interpersonal skills
Experience in HIV care and familiarity with HIV treatment guidelines
Knowledge of community resources and services
Bachelor's degree in Nursing or related field
Bi-lingual/Bi-cultural persons are strongly encouraged to apply
