This is a good read no matter what profession you are in, but especially if you are relatively new in your career. I'm not sure how to teach this, but there has to be an escalation procedure if you see something that concerns you. Start small (friendly chat with the crew???), then work your way up if you're still concerned. Going from zero to full throttle in 3 seconds definitely is not the right approach. Looking back, it was/is nice that in order to get your Professional Engineers License in Ontario, they actually go over cases similar to this (reporting process for engineering safety concerns). Maybe other professions can benefit from something similar. This seems "common sense" to us older folks, but maybe it's not so common to those still starting out. https://lnkd.in/ev5tb96m

New evidence calls into question the NTSB's conclusions – and our reporting in Episode 193 - about a 2020 midair collision. Video and other information shows that there were two helicopters in the area before the midair collision. Miles O'Brien, Todd Curtis, and John Goglia revisit the 2020 midair collision of a drone and a helicopter. The NTSB used a video shot by the drone to conclude that a helicopter seen at the beginning of the video later collided with the drone. When Todd recently used the video as part of a class he was teaching, he noticed a shadow that he could not explain. That led to lots of sleuthing and the realization of the involvement of a second helicopter. Safety concerns arise when one or more helicopters operate around a drone. In addition to this 2020 collision, a midair between two news-gathering helicopters in 2007 that killed everyone on both helicopters. Miles shares his experiences flying in situations where multiple helicopters are covering a breaking news story. Pilots of manned and unmanned aircraft need to coordinate closely to maintain safe operations. The NTSB should consider taking a second look and revising its report to address the aviation safety issues uncovered. John shares his experience on the NTSB Board and what it took to reopen an investigation when new information was available. Addressing the Aviation Maintenance Technician Shortage: A Crucial Step for the Industry’s Future The aviation sector is on the road to recovery post-pandemic, but a significant challenge persists: a shortage of aviation maintenance technicians (AMTs). Current estimates suggest a shortfall of 12,000 to 18,000 AMTs, which could persist or even worsen over the next decade. Factors like an aging workforce, low career uptake, and rising passenger demand (IATA predicts 8.2 billion passengers by 2037) are contributing to this gap. Boeing’s 2021 Commercial Market Outlook indicates that 610,000 new AMTs will be needed globally through 2041, with the trade association ADS saying that there are currently 10,000 vacancies in the UK alone. To meet this demand, the industry must increase the number of trained AMTs by at least 20% above those already being trained. Strategies to address this could include: ✈️ Increasing financial compensation: Making the role more attractive by acknowledging the high skill and training required. ✈️ Diversifying recruitment efforts: Expanding marketing to underrepresented groups (currently, <10% of aviation and aerospace engineers are female, for example). ✈️ Mentorship programs: Leveraging the experience of current professionals to guide and inspire the next generation. I'm going to defend these pilots (not condone) and yep, you might not like it. This was on a CHARTERED flight for the Colorado Rockies. They paid for the entire flight so no other passengers, so let's apply some common sense to both sides. I don't condone their actions, but no one should lose their career over this: Before 9/11, we always left the cockpit door open and often allowed people in the cockpit/flight deck. I flew many sports teams and often, players and managers asked to came up and have a quick look. We'd put the overhead speakers on and brief the passenger on being quiet during radio comms. Of all the "sports teams", the WWF wrestlers (now WWE), were the most respectful and appreciative. They were all required to be in a suit and tie in public (all the sports teams), but everyone took off ties once on board. The WWF wrestlers would put their tie and suitcoat back on before asking to see the flight deck. They'd sit quietly in one of the jumpseats, and were transformed into 12 year old boys. They'd tease me about being a chick in the cockpit and I'd tease them about having too many concussions. I'm a safety advocate and rule follower, but the joy of sharing this amazing moment can still be done safely during a charter flight because if not, you've let the terrorists win again... Pilots - it goes too far to let someone sit in the seat, ever. Period. I wouldn't even let anyone do it on the ground for fear of them hitting a switch. It's a yoga move to get into the seats. All the Part 135 charter pilots here are wondering what the big deal is since passengers have access in many/most twin turbine and bizjet aircraft. Everyone has been scolded, so let's get back to flying airplanes! Alaska Airlines Flight Executes Safe Go-Around Due to Gear Issue, Lands Safely Passengers on a recent Alaska Airlines flight experienced a go-around due to an unsafe gear indication. Flight AS3352, operated by SkyWest Airlines, had departed from Los Angeles International Airport (LAX) and was en route to Eugene, Oregon (EUG) on an Embraer ERJ-175. At 1,300 feet, the pilot initiated a go-around. After working through safety checklists, the plane landed safely 20 minutes later, with passengers unharmed. The FAA reported the incident, noting that a blown tire had damaged electrical wires and paneling related to the landing gear. The flight had departed Los Angeles on July 2 at 08:37, ascending to 34,000 feet and passing over Fresno and the Sierra Nevada. Upon descending into Oregon, the unsafe gear indication was noted. After the go-around, the aircraft landed safely on runway 34L at 10:45 local time. The 6.1-year-old aircraft, configured with 16 business and 64 economy seats, stayed in Eugene for inspection until July 4. It then flew to San Francisco for further checks and returned to service on July 5 without further issues. FAA officials have highlighted the importance of thorough inspections and maintenance, particularly in identifying and rectifying potential hazards. #didyouknow Being out in the dark without proper direction can be frustrating in business and even worse in aviation. Did you know for example that at some airports after published hours, by simply clicking the PTT or Push to Talk button a number of times that the landing lights magically appear? POW! 💥 like a beacon of hope, shining out there from the wilderness, guiding us safely towards yet another destination. Flying at night in the superb Robinson R66 Turbine from Austin, TX to West Palm Beach, FL was certainly an experience—one of many I’ve “enjoyed” during my 25+ years in the industry. Flying can certainly provide a buzz, challenging the senses and requiring thorough planning and making potentially life-and-death decisions in flight. It’s similar to the feeling I get when I’m helping companies steer their businesses towards their goals. Outsourcing remains a pivotal strategy in corporate operations, known for its potential to significantly reduce costs and enhance flexibility in meeting demand fluctuations. However, Boeing’s experience with the 787 Dreamliner project underscores some of the challenges associated with extensively outsourcing critical parts and processes. In an effort to develop the aircraft quickly and cost-effectively to compete with the Airbus A380, Boeing decided to outsource 70% of the design, engineering, and manufacturing of entire modules to over 50 strategic partners. These partners, in turn, outsourced various components to their suppliers. Unfortunately, this layered strategy, aimed at leveraging global expertise and streamlining costs, resulted in significant challenges, including production delays and quality control issues. Drawing from my experience, Apple has seemingly found an ideal balance in its outsourcing strategy. By keeping its product development and core innovations in the U.S., while maintaining a tight grip on its manufacturing processes abroad through robust monitoring systems, stringent supplier qualification processes, and continuous adjustments based on supplier performance, Apple ensures high-quality standards and continuous innovation in its product lineup. This careful integration and monitoring of every step in its supply chain exemplifies a well-executed outsourcing strategy. As companies evolve their outsourcing strategies, learning from these industry examples is crucial. They must craft their outsourcing approaches carefully, focusing on long-term strategic benefits and competitive advantages rather than merely short-term financial gains. In the construction industry, where companies often subcontract most of their work (a process different but related to outsourcing), challenges include ensuring quality, providing consistent feedback to product development teams, and avoiding time or cost overruns. Industrialized construction may address some of these issues. Given these complexities, isn’t it crucial for companies to assess their readiness in terms of processes, establish a robust shared PLM platform with their partners to collaborate seamlessly, and implement watertight monitoring and feedback systems before deciding to outsource?
