We are so inspired by Tracy’s story and the power of Kandu’s program in helping patients navigate and take control of their recovery. Tracy is an opera singer, artist, and activist based in San Francisco. At a pivotal point in her career, she had a #stroke.    Through the support of Kandu, Tracy was able to take control of her recovery and find connection, community and the tools to achieve her goals. Within weeks, Tracy traveled to New York and performed in her career debut at the Metropolitan Opera. Navigating the healthcare system after a stroke can be overwhelming for individuals and their loved ones. In early 2024, the Center for Medicare and Medicaid Services (CMS) introduced a new model for post-acute care called Principal Illness Navigation (PIN). PIN aims to provide individuals with chronic health conditions with holistic support and access to timely and coordinated care as they go through their journey towards recovery. Meet Jason - an athlete, father, and stroke survivor. Jason experienced a stroke last year and spent five days in the hospital. “It was hard to do everything,” Jason shared, reflecting on his new reality after his stroke. Jason immediately immersed himself in the Kandu Stroke Recovery Program and took advantage of what’s offered to stroke survivors– reading educational articles, gaining resources from his Navigator, and connecting with other stroke survivors. “Kandu was a great resource for my recovery,” Jason says, “I could really hone in on what I needed.” Read Jason’s recovery story on our website at https://lnkd.in/gfg6iT3G

We are excited to announce that our peer reviewed paper on the feasibility of Principal Illness Navigation (PIN) services for stroke patients is published in the spring issue of Stroke Clinician, the Official Journal of the Association of Neurovascular Clinicians This study demonstrates the feasibility of enrolling and engaging stroke patients with diverse and complex physical and psychosocial needs in outpatient telehealth PIN services.
