Earlier this month, health centers from across HFP's Health Center Controlled Network (HCCN) came together for the annual HCCN in-person meeting at Normandy Farm Hotel & Conference Center in Blue Bell, Pennsylvania. The HCCN is a grant-supported program open to Federally Qualified Health Centers and look-alikes across Pennsylvania. Many important topics were covered over the two-day meeting, including electronic health records and key components to optimizing these systems; best practices for social drivers of health assessments and referrals; and a discussion on artificial intelligence and other evolving technologies in the healthcare industry. HFP's Diabetes Prevention Program is growing! The year-long Centers for Disease Control and Prevention-recognized educational program will welcome four new groups this summer and fall. Individuals taking part in the program, which is offered in English and Spanish, learn about #diabetes and making healthy lifestyle changes, with topics ranging from fitness and nutrition to managing and identifying stressors. This past week we held our *50th* Peer Models for Health Learning Collaborative session for our peer workforce (Community Health Workers, Certified Peer & Recovery Specialists) partners and their supervisors serving North Philadelphia! This work has allowed us to bring together a fantastic and inspiring network of individuals who do challenging and meaningful work to support the Philadelphia community, and we look forward to continuing to offer opportunities for connection and growth over the next year! National Health Corps, Public Health Leadership is seeking new host sites in Philadelphia for the 2024-2025 service term! Each year we partner with organizations to serve as "host sites." These organizations will have an AmeriCorps volunteer serving 36-40 hours per week for 46 weeks. There is a financial contribution required to support the member's living stipend and operational costs of the program.
