Leaving NYC feeling super inspired and thankful for the problem-solvers of the world. Kristin Wikelius and I had the pleasure of bringing Dr. Meena Seshamani, head of Medicare, to the Cityblock Health Brooklyn facility. Dr. Toyin Ajayi and her team are the living embodiment of problem solvers who take on the hardest issues. We got to hear from the clinical team all of the amazing work they are doing for the most vulnerable patients: how they are able to innovate to continue to solve more and more issues for individuals, and use data and scaled-intiatives to drive change at the population-level. United States of Care is dedicated to directly lifting up the good work of those driving change for policymakers to show that change is possible and where policy can support these innovators. I loved hearing about their mobile-clinic innovation that was born out of the pandemic where they needed to care for people directly in their home. That was such a popular and effective program that they have kept it and now can serve people, when appropriate, across the city in their homes, powered by medics and virtual-providers within hours of a need. Thank you Meena, Toyin, and the whole Cityblock team for what you do, and for sharing the magic with us.

Core to Cityblock’s value-based care model is the integration of physical and behavioral health services. In this newly published peer-reviewed article in Population Health Management, David Dayan-Rosenman, MD, MS and Steven Spencer, MD, MPH, MBA describe the framework and implementation of a structured program that has effectively upskilled primary care providers in medication treatments for substance use disorders, supporting our Medicaid and dual-eligible members with interdisciplinary teams trained in harm reduction.
