The concept relies on moving away from resourcing based on the volume of care delivered to resourcing based on the value of care delivered. The value of care is defined as the patient outcomes divided by the cost of care delivery. Thankfully this message echoed by frontline clinicians are now being realised. Importantly one must note that this is only one part of the answer to overcrowded ED’s and ensuring patients seeking emergent care are treated in time. A change in approach to community access is also important by developing accessible community healthcare clinics / local injury units run by ANPs run between 08:00-20:00 with review and clinical governance by HSE employed generalist GPs. Weekly Telemedicine consultations with appropriate specialist review at these facilities. District hospitals run by HSE employed GPs (who will provide governance of the clinics )for short stay/ IV treatment. For more urgent care patients can be sent to hospital. This will reduce overcrowding of EDs and hospital. The concept of hospital at home should also be encouraged and considered especially for care of the older person. This will make healthcare accessible and cost effective and decrease the serious effects of burnout amongst staff; high state claim payouts and improve safety and quality of care.
